Website inquiry privacy notice.
This public website is for business inquiries. It is not an intake portal for patient, clinical, or confidential client records.
Information you provide.
The inquiry form asks for your name, email address, business, and a description of your reporting needs. We use these details to respond to your request and discuss a potential engagement. Do not include patient details, health records, account credentials, payment information, or private source exports.
Email delivery and retention.
FormSubmit processes website inquiries and forwards them to PB Dataworks by email. Its documentation states that submission copies are retained for 30 days. Copies delivered to our business mailbox are handled separately; they are not automatically deleted when that processor period ends.
Read the FormSubmit privacy terms. Contact us through the inquiry form to request correction or deletion of an earlier inquiry; include only the details needed to identify it.
Website services.
The website is hosted through Sites and uses Google Fonts. Requests to hosting and font providers can include standard connection information such as an IP address and browser details.
Optional website analytics.
With your permission, Google Analytics measures visits to our public marketing pages and counts successfully submitted business inquiries. Google receives standard device and connection details and uses analytics cookies. Approved, non-personal campaign and channel labels help us understand which marketing links bring visitors. Arbitrary URL parameters and free-text campaign labels are not forwarded by our tag configuration. We do not send your inquiry name, email, business, message, or patient information to Analytics. Automatic form measurement and advertising personalization are disabled.
The Google tag does not load until you choose Allow analytics. Choose Decline analytics to use the site and inquiry form without Analytics. You can change your choice using Analytics preferences in the footer. Your choice is stored on this browser for up to 90 days. Analytics cookies are configured for a 90-day expiry; Google Analytics user and event retention is a separate property setting.
